What Each Monthly Audit Includes

• Independent Clinical & Documentation Review — Professional review of your submitted chart sample against applicable regulatory and documentation criteria
• Deficiency Identification — Documentation deficiencies, missing required elements, and non-conforming entries identified and documented
• Compliance-Risk Findings — Findings classified by compliance risk and potential regulatory exposure
• Recurring Pattern Analysis — Identification of patterns across the monthly sample indicating systemic documentation issues

Standard Turnaround

Engagement Terms

• Unused chart volume within a given month does not roll over
• Additional charts beyond your selected tier, expanded scope, or ad hoc requests are separately scoped and priced
• A scope alignment intake is completed prior to the first audit cycle
